Comments:
An unannounced monitoring visit was conducted today in the afternoon. Observed children sleeping in the three classrooms, physical space, transition to afternoon snack and preparation for dismissal. Ratios with direct care staff was maintained in all rooms, emergency drills and trainings were complete and space is well maintained and equipped with age appropriate materials and equipment. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-4
Description: Based on record review and center administration statement, a Central Registry Search for a staff member had not been completed within 45 days for a staff member who has lived outside the State of Virginia within the past five years.
Evidence:
Staff #1 signed a Sworn Disclosure Statement that they have lived outside of Virginia within the past 5 years. No CPS search was obtained within the 45 day requirement. Staff was hired 8/30/2022.

Plan of Correction: An out of state CPS search will be submitted and staff will not return until a completed search is on file.

Standard #: 8VAC20-780-40-M
Description: Based on staff statement a written list of all children's allergies, sensitivities and dietary restrictions was not kept in each room or where children are present.
Evidence:
The current list did not include dietary restrictions.

Plan of Correction: Additional information will be added to the list to ensure all dietary information is listed.

Standard #: 8VAC20-780-510-E
Description: Based on record review and center statement, 2 medications do not have physician signatures for medications on-site longer than 10 days.
Evidence:
Medication for Child #1 and Child #2 did not have long term authorizations signed off by the physician for medication on-site longer than ten days.

Plan of Correction: Medication will be returned to the parent unless the authorizations are signed off by the doctor after ten days.
