An unannounced complaint inspection was conducted on-site November 10, 2022. The department received a complaint regarding allegations in the area of supervision. The director was available during the inspection.
The evidence gathered during this investigation did not support the allegation of non-compliance with standards or law and no violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 8VAC20-780-130-A Complaint related: No Description: Based on a review of children's records on November 10, 2022, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: Child #1, was enrolled 8/16/21, prior to the center obtaining documentation of the child's immunizations.Plan of Correction: New student records need to be reviewed by at least two administrative staff to ensure records are in compliance with standards prior to enrollment. Child #1's vaccination administration record was updated 9/17/21 and added to the file on record. Records will be reviewed prior to enrollment and checked by at least 2 staff members.
Standard #: 8VAC20-780-280-B Complaint related: No Description: Based on observation on November 10, 2022, the center failed to ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence: In the upstairs hall bathroom used by children there was Lysol spray, disinfectant wipes, and Glade on a shelf. The labels on each of the products contained the language "keep out of reach of children" and "caution."Plan of Correction: Install locking cabinet in restroom. Inspect all rooms on a regular basis to ensure potentially hazardous substances are secured safely. Hazardous substances were moved out of inspected bathroom until a new cabinet with child lock was installed on 11/19/22.
