Technical Assistance:
Technical assistance was offered related to: Background checks, staff orientation training, Medication forms, and special order issued for the center.

Comments:
An unannounced monitoring inspection was conducted on 11/28/22. There were 32 children present, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed playing outside on the playground, and enjoying singing using a karaoke machine. 4 staff records, 3 children's records, emergency drills and medications were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-80-A
Description: Based on records review and staff interview, the center did not maintain a written record of arrival and departure for each child as it occurs.
Evidence:
1. In the Hershey's classroom, there were 9 children present, but only 7 children's arrival was marked.
2. Staff # 1 stated that Child # 1 had arrived around breakfast time; and Child # 2 had arrived at approximately 9:00 am.
3. Child # 1 and Child # 2's 's arrival was not documented on the attendance record.

Plan of Correction: The staff member responsible was coached on the seriousness of this violation. She was also suspended from work and placed on probation pending termination if this violation occurs again.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment inside the center was not maintained in a safe and operable condition.
Evidence:
1. One of the two bathroom sinks used by the Hershey's classroom was detached from the wall and partially hanging from the wall.
2. The cushion for the rocking chair in the infant classroom had a large tear in the corner, exposing the inside filling.

Plan of Correction: A contractor was called the day sink came apart from the wall. He came to repair it on 11/23. Said he needed parts and will be back to repair it when parts come in.

Standard #: 8VAC20-780-280-B
Description: Based on observation, cleaning materials were not kept locked and out of reach of children.
Evidence: In the Hershey's classroom (3 years old) a bottle of bleach and water solution was found on the shelf adjacent to the bathroom. The bottle was within reach of the children in the classroom.

Plan of Correction: The bottle was locked away during the inspection.

Standard #: 8VAC20-780-330-E
Description: Based on observation, a sandbox was not covered when not in use.
Evidence: The cover for the large sandbox on the playground had several tears, and was not covering the sand box from the corners and outer edges. The sand box was not in use by the children.

Plan of Correction: A new tarp was ordered Due to arrive 12/1. Replaced on 12/1.

Standard #: 8VAC20-780-500-B
Description: Based on observation, a diapering surface was not non-absorbent.
Evidence: In the infants classroom, the diaper pad had small tears making it an absorbent surface.

Plan of Correction: The diaper pad was replaced with a new one.
