Discussed with the provider: MAT training for emergency medications, children?s files, terms of license, inspection report posting requirements

Comments:
An unannounced Renewal Inspection took place on 11/22/2022 between the hours of approximately 9:10 a.m. and 11:30 a.m. There were 4 classrooms observed with a total of 25 children with 7 staff within the supervision guidelines. The children were observed eating breakfast (pancakes, applesauce and milk), practicing writing their names and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-130-A
Description: Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child could attend the center.

Evidence: Child #1 (start date: 11/02/2022) did not have documentation of immunizations on file.

Plan of Correction: We will get it from mom.

Standard #: 8VAC20-780-140-A
Description: Based on record review, not each child had a physical examination before the child?s attendance or within 30 days after the first day of attendance.

Evidence: Child #2 (start date: 09/13/2022) did not have documentation of a physical examination on file.

Plan of Correction: The child has an appointment on 11/28. We will get a copy then.

Standard #: 8VAC20-780-40-E
Description: Based on observation and record review, the center did not ensure compliance with the terms of the current license.

Evidence:
1. Child #1 (age: 7 weeks) was observed in the infant room. Based on record review, the child began care at the center on 11/02/2022 at the age of 4 weeks.

2. The center is currently licensed to provide care for children ages 2 months- 12 years old.

Plan of Correction: Will not enroll any new children until 2 mos or until the license is changed to 1 month.

Applied to change license from 1mo- 12 years.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. There was chipped paint on the red baseboard by the blue couch in the Two?s classroom.

2. There was peeling black contact paper on the back of the cubbies in the Two?s classroom.

3. The tile on the floor of the bathroom, located in the Two?s classroom, was chipped.

4. The tile on the floor of the girls? bathroom by the Three?s classroom was chipped.

5. The blue jungle gym on the playground used by the threes, fours and school-agers, had chipped paint and rusting metal.

Plan of Correction: I will put in a work order today.
