Comments:
A monitoring inspection was initiated on 11/15/2022 and concluded on 11/15/2022. There were 65 children present, ranging in ages from 3 months to 5 years, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 5 medication authorizations and 4 staff records were reviewed/updated. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of staff records, the center failed to ensure that tuberculosis screening/test was within 30 days of employment.

Evidence:
1. Staff #2's date of employment was 11/7/2022. The tuberculosis screening/test on file was dated 09/13/2022.
2. Administration verified the date of employment for Staff #2 and the date of on the tuberculosis screening/test.

Plan of Correction: Going forward administration will pay attention to TB dates when hiring new staff members.

Standard #: 8VAC20-780-320-B
Description: Based on observation and measurement, the center did not ensure to have sinks located near the toilets and that are supplied with running warm water that does not exceed 120 degrees F.

Evidence:
1. The water temperature was measured using a digital thermometer. The water temperature reading on the thermometer in the 3's room was 133.7 degrees F and in the K4 room it was 135 degrees F.

Plan of Correction: Director will turn back the temperature on the hot water to 120 degrees F.

Standard #: 8VAC20-780-510-E
Description: Based on review of the centers medication policy, the center failed to ensure that there is a policy to include methods of preventing use of outdated medication.

Evidence:
1. Child 3M EpiPen expired 5/2022.
2. The medication policy does not discuss discarding or returning expired medications to prevent use of outdated medications.

Plan of Correction: Medication policy was updated with how to return or discard expired medication.
