Accurate record of children with each staff as classes use the elevator to leave the third floor for an activity.

Comments:
An unannounced monitoring inspection was conducted today. All groups were in ratio during the inspection.
5 staff and 4 child records were reviewed. See Violation Notice for non-compliances sited.
Medications, handwashing, child activities and activity transitions were all observed today.

The inspection report will be typed off site and emailed to the facility for signatures. The LI was present today from 10:57 AM - 1:55 PM.

Standard #: 22.1-289.035-B-2
Description: Based on a review of records, one staff record does not contain documentation of the results for a nationwide fingerprint search prior to employment.
Evidence:
The fingerprint results available for review are dated 8/10/22. (Staff A: DOH: 8/1/22).

Plan of Correction: Beginning 12/2/22 all future hires will complete their fingerprints search with the date prior to their hire/start date.

Description: Based on a review of records, staff records do not include:
*A completed Sex Offender search for one staff member that has resided outside of the state of Virginia in the past 5 years prior to employment.
*A completed Child Abuse and Neglect Search within 45 days of employment, for 2 staff that have resided outside of the state of Virginia in the past 5 years.

Description: Based on a review of records, one staff record does not include documentation of references prior to employment.
Evidence:
1 of 5 records reviewed did not contain any references having been checked. (Staff A: Date of Hire (DOH) 8/1/22)

Plan of Correction: References for Staff A have been documented and now available in the staff file.

Standard #: 8VAC20-780-210-A
Description: Based on a review of records, one staff record does not include documentation of qualifications.
Evidence:
1 of 5 staff records reviewed did not contain documentation of lead teacher qualifications. The record state the staff person is in a lead position. (Staff A)

Plan of Correction: Staff A is not regarded a classroom lead and that error has been corrected on the Staff file checklist which listed the staff member as a lead.

Description: Based on a review of records, orientation documentation available for review does not contain all required components.
Evidence:
5 of 5 records reviewed contained an orientation form used for staff is outdated and does not document all required items.
Missing items include: Prevention of Abusive Head Trauma, prevention of and response to allergic reactions, prevention and control of disease, emergency preparedness and response planning. (Staff A, B, C, D,E)

Plan of Correction: Outdated forms have been replaced with the VDOE model form (10/21-3).
All staff have been reviewed and signed off on the most up-to-date orientation form including :Prevention of Abusive Head Trauma, prevention of and response to allergic reactions, prevention and control of disease, emergency preparedness and response training.
