12:55-2:15pm Annual unannounced subsidy inspection conducted. Observed 2 children = 4 points with the vendor. Ratios and supervision were in compliance. Children were napping during the inspection. The home was found to be clean and well supplied with toys and equipment for the children. The vendor is currently a home-based Head Start provider. Annual training requirements completed. First Aid/CPR certifications current. Areas of non-compliance found.

Standard #: 22VAC40-665-170-B Description: Based on records reviewed, it was determined that 2 of 4 children's records did not include required information.
Evidence:
1. Child #9 - 1 of 2 parent name, home/work addresses and phone numbers. The space on form was left blank.
2. Child #11 - 1 of 2 parent name, home/work addresses and phone numbers. The space on form was left blank.Plan of Correction: The vendor said she will have the parent complete the form.
Standard #: 22VAC40-665-240-C-2 Description: Based on observations made, it was determined that electrical outlets did not have protective covers. Evidence: 3 electrical outlets, in areas used by the children, did not have covers today - 1 outlet by the kitchen table where they eat lunch, and 2 outlets in the bathroom. Plan of Correction: The vendor covered the outlets during the inspection today.
