Comments:
An unannounced monitoring inspection was conducted today from 2:18pm-3:30pm. There were 46 children (school-age) directly supervised by 4 staff. The physical plant, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Staff records review is completed annually at the administrative office through the County of Loudoun, Department of Parks, Recreation, and Community Services. Children were observed participating in group play, working on their homework, discussing giving back to the community and receiving student of the month awards. There was an adequate supply of materials for the children. The program was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Description: Based on record review, the program did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child could attend the center.

Evidence:
Child #1's file was missing the child's immunization record.

Description: Based on review of the emergency preparedness plan and interview with staff, the center did not ensure that the emergency preparedness plan contained all required information.

Evidence:
The emergency preparedness plan was missing the evacuation relocation site and the shelter-in-place and lockdown assembly points.
