Comments:
An unannounced renewal inspection was conducted on-site December 5, 2022. The director was available during the inspection. There were 38 children present, ranging in ages from 3 months to 4 years, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on a review of children?s records and interview on December 5, 2022, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: The record of child #2, enrolled 11/28/22 contained documentation of immunizations dated 11/30/22. Staff #2 confirmed the immunizations were late.

Standard #: 8VAC20-780-270-A
Description: Based on observation on December 5, 2022, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: The preschool room contained chipped and peeling paint on several walls. There were also a vertical line of several holes on the back wall that was approximately a foot long.

Standard #: 8VAC20-780-510-P
Description: Based on review of medications and authorizations on December 5, 2022, the center failed to ensure when an authorization for medication expires, the parent was notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.
Evidence: 1. The medication authorizations for child #6 expired 7/24/22, the medication had not been returned to the parent nor had the parent renewed the medication authorization form.

Standard #: 8VAC20-780-570-A
Description: Based on observation and interview on December 5, 2022, the center failed to ensure when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.
Evidence: 1. In the infant room, six infants were observed sitting in bucket seats in the feeding/activity table. 2. Five of the infant?s protective belts were not fastened and one child did not have a protective belt. 3. Staff #1 in the infant room acknowledged the protective belts were not fastened and stated she has told the other staff in the room to ensure they are. 4. In the toddler room, two children were observed in high chairs without the protective belts fastened.
