An unannounced non-monitoring inspection was conducted on 11/29/2022 and 11/30/2022 regarding self-report received on 11/18/2022. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record and 3 staff records were reviewed. Interviews were conducted in conjunction with local Child Protective Services.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-770-60-C-2 Description: Based on review of records and interview, the center failed to ensure that Central Registry results were received by the end of the 30 days of employment.
Evidence:
1. Staff #1 start date on file was 3/24/2022.
2. The record showed the central registry was sent on 4/21/2022. Staff #6 confirmed the central registry check had been sent within 7 days of employment.Plan of Correction: Management will send the CPS Background check out within the first week of employment.
Standard #: 8VAC20-780-400-A Description: Based on interviews, the center failed to provide needed help, comfort and support to a child.
Evidence:
1. A relative of child #1 reported to the center that she observed, on the center?s camera system, Child #1 crying and being ignored by Staff #5.
2. Interview with Staff #4 confirmed that Child #1 was crying and ignored by Staff #5, as witnessed by Staff #4.
3. The CPS Representative stated that during an interview with Staff #5, Staff #5 admitted to being irritated with and ignoring Child #1.Plan of Correction: Providing additional training on Abuse and neglect by 12/14/2022 and offer additional breaks as needed.
