Technical Assistance:
1. Reviewed record retention requirements.
2. Forms provided: daily attendance, and emergency contact.

Comments:
10:10-11:15am Annual unannounced subsidy inspection conducted. Observed 1 child = 4 points with the vendor. Ratios and supervision were in compliance. There were 2 household members present. The home was found to be clean and sufficiently supplied with toys and equipment for the children. The child was playing with toys, being held, and watching a child appropriate cartoon on tv. The vendor completed the annual training requirements, and is currently certified in First Aid/CPR. Areas of non-compliance found.

Standard #: 22VAC40-665-170-B
Description: Based on records reviewed, it was determined that 1 of 3 children's records did not include required information.

Evidence:

1. Child #4 - 2 of 2 parent home/work addresses, and the address for the emergency contact person listed.

Plan of Correction: The vendor said she will have the parent write this information on the form.

Standard #: 22VAC40-665-260-A-3
Description: Based on observations made, it was determined that the hot water temperature exceeded 120 degrees. Evidence: The hot water temperature was measured today to be 140.1 degrees.

Plan of Correction: The vendor has not control over the water temperature in the building, and said that she stays with the children when they are washing their hands to be sure the water is not too hot.
