Comments:
A unannounced mandated inspection was conducted on December 7, 2022 from 10:05 A.M.-1:15 P.M. There were 49 children present, ranging in ages from five months to five years, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and eleven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-E
Description: Based on review of children records, the center failed to obtain additional immunizations once every six months for children under the age of two years.

Evidence:
1. Child #5,(age 20 months), last immunizations on file were dated 11/19/2021. Updated immunizations were required in May 2022, and again in November 2022.
2. Administration verified that there were no updated immunizations obtained for Child #5.

Plan of Correction: Send out reminders informing parents that updated immunizations are needed 60 days before they are required, again at 30 days, 14 days, and one week. This will ensure that families are aware of the required documents needed with plenty of time to work out any problems that might arise before documents are due to us at the center. Updates have been requested to the parent for a copy of the updated immunizations.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the center failed to ensure that staff washed their hands at all required times.

Evidence:
1. In Classroom #1, Staff #1 was observed preparing for and completing a diaper change. Staff #1 placed a pair of gloves on her hands, and placed the child on the changing table and proceeded with changing the child's diaper. Staff #1 did not wash her hands before putting the gloves on and starting the diaper change.
2. Staff #1 verified that she did not wash her hands before completing the diaper change.

Plan of Correction: Review diapering and handwashing procedures with staff.
