Comments:
Today an unannounced Monitoring Inspection took place between the hours of 3:35pm and 4:45pm. There were cafeteria and gym areas were observed with a total of 49 children with 4 staff within the supervision guidelines. The children were observed playing dodgeball in the gym, making arts and crafts and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, emergency medications, first aid kit, and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Description: Based on observation, staff-to-children ratios were not being followed while children were
in care.

Evidence:
1. At approximately 3:35pm, the Licensing Inspector arrived to the facility and observed that Staff A was supervising 25 children in the cafeteria.
2. The LI asked if there was another staff member in the cafeteria and Staff A stated "it is my first day working here, there was another staff member here but they took children to the bathroom. I am not sure what their name is since I just started."
3. At approximately 3:39pm, Staff B entered the cafeteria from the hallway with two children who Staff B had taken to the bathroom.
4. The ratio for the school-age children in the program is 1:18. Staff A was out of ratio for approximately 4 minutes.

Plan of Correction: We will make sure we always stay in ratio.
