Technical Assistance:

1. The most recently issued violation notice is to be posted in the facility for public viewing. 2. Reviewed out-of-state background check requirements. Inspector provided guidance and resources.

Comments:

Conducted an unannounced inspection at 10:50am. Observed 49 children + 11 direct-care staff. Ratios and supervision were in compliance. Children were observed in a variety of activities to include: painting and other art projects, playing with toys and blocks, counting numbers, indoor and outdoor active play. The infants were playing with toys. Staff were actively engaged with the children and activities. Lunch served today: beef, cheese, corn, tortilla chips (nachos), pears, and milk. The facility was found to be clean and well supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Standard #:	22.1-289.035-B-4
Description:	Based on records reviewed, it was determined that 1 of 4 staff records did not include documentation of required out-of-state background checks within the first 30-days of employment. Evidence: Staff #2 was hired 6/25/21 and to-date there was not a sex offender registry check completed, or a child abuse/neglect search, as required for the given State.
Plan of Correction:	The sex offender registry check was completed during the inspection. The child abuse/neglect search will be submitted to the given State promptly.
Standard #:	8VAC20-780-160-A
Description:	Based on records reviewed, it was determined that 1 of 4 staff records did not include documentation of a TB test/screening at the time of employment and prior to coming into contact with children. Evidence: Staff #2 was hired 6/25/21 and there was no TB screening on-file.
Plan of Correction:	TB screening completed.
Standard #:	8VAC20-780-70
Description:	Based on records reviewed, it was determined that 1 of 4 staff records did not include documentation of the education needed to qualify the staff person for the position she is working. Evidence: Staff #4 is currently working in a Lead Teacher position, but education document(s) required were not on-file.
Plan of Correction:	The education document(s) will promptly be obtained and placed in the Staff 4's record.
Standard #:	8VAC20-780-280-B
Description:	Based on observations made, it was determined that a hazardous substance was not kept in a locked place when not in use. Evidence: A spray air freshener product was found on an open shelf in a bathroom used by children ages 2-3 years old.
Plan of Correction:	The product was stored properly during the inspection.
