A monitoring inspection was initiated on 12/27/2022 and concluded on 12/27/2022. There were 20 children present, ranging in ages from 8 months to 5 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 7 staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-780-160-A Description: Based on record review, the center failed to obtain documentation of a negative tuberculosis screening/test completed within the 30 calendar days prior to the date of employment for all staff.
Evidence:
1. Staff #1 date of hire 08/22/2022. The tuberculosis (TB) screening on file was completed 06/06/2022.
2. Staff #2 date of hire 09/06/2022. The TB screening on file was completed 09/24/2022.
3. Staff #3 date of hire 10/10/2022. The TB screening on file was completed 11/16/2022.
4. Staff #6 date of hire 08/10/2022. The TB screening on file was completed 08/14/2021.
5. Administration confirmed the dates of hire and the dates of the TB screenings for all the staff.Plan of Correction: We were having a hard time in our area getting the TB screenings. So one of our parents (an RN) is going to be completing the TB screenings now prior to hire.
