Comments:
An unannounced subsidy health and safety inspection was conducted on 12/27/2022 from 10:50 AM - 12:40 PM. At the time of entrance there were 21 school age children in care with 6 staff. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-600-D-1
Description: Based on record review and interview, the vendor failed to ensure that all staff who work directly with children shall have within 90 days of the date of employment current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Evidence:
1. The following staff lack documentation of current CPR certification:
a. Staff 1 - hire date 06/18/2022.
b. Staff 2 - has been employed for several years according to administrative staff.
c. Staff 3 - CPR expired 06/14/2022.
d. Staff 4 - hire date 06/20/2022.
e. Staff 5 - hire date 06/20/2022.
2. Administrative staff confirmed that CPR certification was not on file for staff 1, 2, 4 and 5. CPR for staff 3 has not been updated.

Plan of Correction: Administrative staff stated that all staff would complete CPR and First Aid if their certificates could not be located. All staff completed First Aid/CPR certification however, the instructor has not provided certificates.
