Comments:
An unannounced renewal inspection was initiated and concluded on 1/09/2023. The inspector was on site from approximately 9:19 am-10:55 am. There were 31 children present, ranging in ages from 4 months to 5 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition and background checks. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-520-C
Description: Based on observation and interview, the center did not ensure that when diaper ointment or cream is used, the requirements were met.

Evidence:
1. Aquaphor was observed that was not labeled with the child's name as required.
2. Administration acknowledged that the Aquaphor diaper ointment was not labeled.

Plan of Correction: Teachers will label all ointment and notify administration about any ointment being used before taking it from parents.

Standard #: 8VAC20-780-560-G
Description: Based on observation and interview, the center did not ensure that when food is brought from home, the container was clearly dated and labeled in a way that identified the owner.

Evidence:
1. In the refrigerator, two bags were observed that contained milk bottles from home. The container and bottles were not labeled or dated in a way to identify the owner.
2. Administration acknowledged that the bottles were brought from home for the children and were not labeled or dated.

Plan of Correction: The opening staff will make sure all bottles brought from home are labeled with date and name before putting the bottles in the refrigerator. Staff will do a refresher training on infant care.
