An investigation was initiated from 10:45 am - 12:00 pm to investigate a self reported incident regarding supervision and care, protection, and guidance. Interviews were conducted, documentation was reviewed, and video footage was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-340-A Description: Based on interviews and review of video footage it was determined that staff did not always ensure the care, protection, and guidance of children in their care. Evidence: On 12/19/2022 Child A (age 3 yrs) was left alone in the STEAM Classroom from approximately 3:50 pm - 4:05 pm. During the time child A was in the STEAM classroom they are seen trying to open the door, kicking the door, and at one point got their hand stuck in the door while trying to open the door. Child A was later found by a parent who was entering the facility and returned to administration. Staff in the toddler classroom were not aware that child A was not in their care from the time of 3:50 pm - 4:05 pm. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-340-F Description: Based on interview and review of video footage it was determined that children under 10 years were not always within sight and sound supervision of staff. Evidence: On 12/19/2022 the toddler classroom was leaving the STEAM lab classroom to go back to the Toddler classroom. There were 10 children in care with 2 staff supervising. Children began to line up by the door to exit. Staff were assisting two other children who were running away from the door. The children and staff began to exit. Child A (Age 3 yrs) goes behind the door and is not seen by staff who exit with the children. Child A was left alone in the STEAM classroom from approximately 3:50 pm - 4:05 pm.
