Comments:
An unannounced monitoring inspection was conducted on 1/11/23 from 7:30am to 9:40am with program staff then the Director and Assistant Director. There were eight children in care at the beginning of the inspection, ranging in age from two-years to five-years-old, supervised by one staff as the cook prepared breakfast. More children and more staff arrived as the inspection progressed. The children were observed having free play in centers then cleaning up and washing hands for breakfast, then eating breakfast. Five child records and four staff records were reviewed. The Center has seven staff with current certification in CPR and First Aid, and four staff trained in Daily Health Observation Training. One medication and authorization form were reviewed and the center has five staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 5/26/22 and the most recent Health Inspection on file was dated 10/27/22. Breakfast served was cheesy hashbrown potatoes, blueberries, and milk. The Director stated that the playground is currently closed as they are getting estimates for needed repairs. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 1/19/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-780-70
Description: Based on review of four staff records, the center did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment for two staff. Evidence: The records for Staff #1 and Staff #4 did not contain documentation of references.

Plan of Correction: References will be checked when staff are sent to complete a background check.

Standard #: 8VAC20-780-245-L
Description: Based on observation and review of staff records, the center did not ensure that there was always at least one staff member on duty who had obtained within the last three years instruction in performing the daily health observation of children.

Plan of Correction: We are getting everyone DHO trained, while seeing who had and did not have DHO, we did notice that there was one teacher on staff with DHO training at the time of the inspection. We are adding DHO to our new hire checklist to be completed within the first 30 days of employment.

Standard #: 8VAC20-780-350-B-3
Description: Based on observation and record review, the center did not ensure that the required ratio of staff to children was met. Evidence: On 1/11/23 from approximately 7:59am to 8:04am there was one staff member present with 9 children between the ages of two-years-old and five-years-old. The required ratio with two-year-olds present is 1 staff to 8 children.

Plan of Correction: Train staff to alert person in charge as soon as they are over ratio, training staff not to accept children into the classroom if the child is going to place that teacher over ratio.
