Comments:
A monitoring inspection was initiated on January 18, 2023 and concluded on January 18, 2023. There were 20 children present, ranging in ages from 12 months to age 4, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and staff interviews, the licensee did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence: On the playground there were two pieces of play equipment that pose a safety hazard to the children.
1) On the school age playground there is a wooden ship that has 5 areas of chipped and peeling paint ranging in size from approximately 6 inches to 3 feet, a 3-foot area approximately where the wood is splintering and there are 6 nails/screws that are protruding from the ship.
2) On the toddler playground there is small plastic composite piece with a slide that has an approximate 6-inch crack, at the top of the slide.

Plan of Correction: Plan of correction for Boat: Children were and are not allowed on the boat. Caution tape has been put on it until it is repaired. Owner has given a date of 3/1/2023 (6 weeks for repairs to be made)
