Comments:
A monitoring inspection was conducted on January 18, 2023. There were 42 children present, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of staff records, each staff member did not submit documentation of a negative tuberculosis screening by the first day of employment dated within 30 days prior.
Evidence
The tuberculosis screening for staff #1, first day of employment 2/9/2022, is dated 6/3/2021, which is not within 30 days of first day of employment.

Plan of Correction: We will make sure all TB screenings are received by the first day of hire and were not completed more than 30 days prior.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances were not kept locked using a safe locking method that prevents access by children.
Evidence
A cabinet located in the main hallway that contained hazardous cleaning substances was unlocked making the items accessible to children.

Plan of Correction: We will make sure all cabinets are locked, this one isn't working properly, the maintenance man is here today to fix it.

Standard #: 8VAC20-780-370-5
Description: Based on review of infant daily activities, documentation for infants who cannot turn themselves over and are awake, being placed on their stomachs a total of 30 minutes each day to facilitate upper body strength is not being recorded.
Evidence
No time was recorded of infants being placed on their stomachs for infants who cannot turn themselves over and are awake.

Plan of Correction: Staff began recording the tummy time today and will continue to do so.

Standard #: 8VAC20-780-440-B
Description: Based on observation, cots were not identified for use by a specific child.
Evidence
In the Infant B room, there were more children present, than labeled cribs.
The cots located in the Two's classroom, were numbered but no corresponding list of names was available.

Plan of Correction: The cot name to number list will be done today. The infant is new, we will make a name sign for them. Going forward we will make sure all cots and cribs are easily identifiable.

Standard #: 8VAC20-780-500-B
Description: Based on observation and discussions with staff, the diapering surface is not being cleaned and sanitized according to licensing standards.
Evidence
The diapering surfaces in the Infant B classroom and the Two's classroom are not being cleaned and sanitized using the procedure required by the Virginia Licensing Standards. The diapering surface must be cleaned by spraying or dipping with a solution of soap and water, pat dry, then spray or dip with a solution of bleach and water to sanitize and allow to air dry.

Plan of Correction: We will retrain staff again on the proper procedure and monitor to ensure it is being followed.

Standard #: 8VAC20-780-520-A
Description: Based on review of over the counter skin products, the center kept one beyond the expiration date of the product.
Evidence
In the Two's room an over the counter skin product was located in the diapering area for child #1, with an expiration date of 8/2022.

Plan of Correction: The expired topical will be disposed of, and we will make sure all products are not expired.

Standard #: 8VAC20-780-520-C
Description: Based on review of diaper ointment or cream, not all products are labeled with a child's name. Diaper ointments or creams were located in an area that is accessible to children.
Evidence
Diaper ointment or cream in the Infant B classroom was not labeled with a child's name.
Diaper ointments or creams in the Infant A classroom, the Two's classroom, and the Toddler's classroom were located in the cabinet under the diapering area that is accessible to children. The cabinet does not need to be locked, but the ointments or creams need to be inaccessible to children.

Plan of Correction: We will make sure all topical are labeled.

Standard #: 8VAC20-780-550-D
Description: Based on review of the monthly practice evacuation drill log, the center did not implement a monthly practice drill.
Evidence
The is no documentation of a monthly practice evacuation drill for the months of April through December 2022.

Plan of Correction: Going forward we will make sure to record all monthly evacuation drills.

Standard #: 8VAC20-780-550-P
Description: Based on review of the center's written record of children's serious and minor injuries, the records are not complete.
Evidence
The accident reports reviewed from April 2022 to January 2023, not all contained the required information including: obtaining two signatures, date and time of accident, time and method parent was notified, and child's full name.

Plan of Correction: Going forward we will make sure all information that is required is on the forms. We will retrain staff on completing the forms.

Standard #: 8VAC20-780-560-G
Description: Based on observation, when food was brought from home it did not always include the child's full name or current date.
Evidence
In the Infant B classroom, there were four cups in the refrigerator that were not clearly dated and labeled in a way that identified the owner; Staff confirmed the cups were full when brought in from home.

Plan of Correction: We will make sure when something is brought is from home it is clearly labeled with the date and child's name.
