The licensing inspector conducted an unannounced complaint inspection on 01/23/2023 In response to a complaint that was received in the licensing regarding allegations in the area of supervision. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-350-Q Complaint related: Yes Description: Based on interviews and a record review the center did not ensure all supervision requirements were met.
Evidence:
In December, the Interim Director assigned a 3 year-old child #3 into the Pre-K class with 4 and 5 year-old children. The Interim Director did not obtain the written permission of the parent nor a written assessment by the program director and program leader, documenting if the Pre-K group was appropriate for the child's development level.Plan of Correction: Plan of Correction: Parent signed transition letter on 01/20/2023 permitting their child to move into the Pre K program. Assessment was done in November showing child was developmentally ready to transition to Pre K program. Moving forward although curriculum and programing in the Pre K program is appropriate for a 3 year old, going forward we will review and sign off on transitions with parents when moving children to new classrooms.
