An unannounced monitoring inspection was conducted on January 31, 2023 from approximately 11:30 am-1:16pm. There were 10 children present at this time with 4 staff members. Additional staff was on site for administrative and support services. Children were observed eating lunch.
Three staff records, three children's records, 3 injury report and 2 medications were reviewed during the inspection. Licensing Inspector reviewed indoor areas, supervision, activities, equipment, medication
procedures, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on
the violation notice issued to the program.

Standard #: 8VAC20-780-160-C Description: Based on record review, the licensee did not ensure at least every two years from the date of the first initial T.B. test screening staff members should obtain and submit the results.
Evidence : Staff #1 most recent tb test was 11/23/2020 more than 2 years old.Plan of Correction: Staff #1 had a TB test completed on January 31, 2023.
