An unannounced monitoring inspection was conducted on 1/26/2023 from 10:26 to 12:10pm. There were 80 children (Infant to 5yrs) directly supervised by 13 staff. The physical plant, 4 staff records, 5 children?s records, 4 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 8VAC20-780-160-C Description: Based on review, the facility did not ensure that at least every two years from the date of hte first initial screening or testing, or more frequently if recommended, staff members shall obtain and submit results of a follow-up TB screening.
Evidence: reviewed 4 staff records and found that Staff 3 and 4 did not have TB tested completed within 2 years of the first initial screening.Plan of Correction: Corrected on 1/29/2023
Standard #: 8VAC20-780-270-A Description: Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: observed the following:
1) in the Twos classroom, observed two holes in the wall at the far end of the room.
2) in the Toddler classroom, observed tears and cracks in the carpet and flooring causing a tripping hazard.Plan of Correction: Maintenance has been contacted and it will be corrected by the end of March.
Standard #: 8VAC20-790-550-2-d Description: Based on review, the facility did not ensure that staff had current sworn statements on file.
Evidence: Reviewed 4 staff files and found that Staff 4 did not have documentation of a current sworn statement. It was last signed on 1/5/2018.Plan of Correction: Corrected at the time of the inspection.
