Technical Assistance:
Discussed with Director: all staff should be aware of the allergies in each room, including location of the allergy list.
DOE will continue to pay for fingerprinting through June 2023.

Comments:
A monitoring inspection was conducted today from1:56PM - 3:50 PM.
All rooms were in ratio during the inspection.
The Toddler playground, emergency evacuation drills, injury reports, medications, rest cot placement and top and bottom covers were all observed and in compliance.
Children were observed resting quietly, being fed and playing with age appropriate toys and games.
6 staff and 5 child records were reviewed. See Violation Notice for non-compliances cited.

Standard #: 8VAC20-780-160-A
Description: Based on a review of records, staff records do not contain documentation of negative tuberculosis screening dated within 30 days prior to the date of employment.
Evidence:
*Staff 5: Date of Hire(DOH) is documented as 10/17/22. The screening results on file are dated 10/18/2022. Not prior to employment.
*Staff 6 (DOH: 10/3/22) Screening results on file are dated 8/22/22, more than 30 days prior to employment.

Plan of Correction: Staff will not hired until the TB is correctly documented.

Standard #: 8VAC20-780-60-A
Description: Based on a review of records, child records do not contain documentation of all required components.
Evidence:
2 of 5 child records reviewed did not contain compete emergency contact information.

Plan of Correction: Admin reached out to get the information. Moving forward, the children will not start until the file is complete.

Standard #: 8VAC20-780-240-A
Description: Based on a review of records, staff records do not contain documentation of the Virginia Department of Education sponsored orientation course within 90 days of employment.
Evidence:
Staff 4 (DOH) 8/8/22 does not have documentation on file to show the required 10 hour orientation course was completed.

Plan of Correction: Admin will make sure an account will be established to start the course on the the first day. Follow ups will be done to make sure the course is being completed in a timely manner. Staff #4 had the course completed; however, the certificate was found until after the inspection was completed.
