Comments:
A renewal inspection was conducted on 2/13/23 from 10:30am until 12:10pm. At the time of the inspection, there were 30 children in care with 6 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed participating in learning activities, playing outside on the playground and eating lunch. Lunch service diapering procedures and handwashing and restroom procedures were also observed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports, medication and required center postings were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations were documented on the violation notice issued to the program and discussed with the center director during the exit interview.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review and interview, the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening that shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
1. The record for staff 1 (date of employment: 1/10/23) contains documentation of a negative tuberculosis screening dated 5/5/22, which is not within the last 30 calendar days of the date of employment.
2. The center director confirmed that the tuberculosis screening for staff 1 was not completed within the required timeframe.

Plan of Correction: The center responded with the following: Staff 1 will obtain an updated TB screening by 2/14/23. Going forward, the center will ensure that new staff obtain TB screenings within the required timeframes.

Standard #: 8VAC20-780-260-B
Description: Based on a review of center records and interview, the center did not ensure that annual approval from the health department shall be provided.

Evidence:
1. Documentation of the most recent health inspection for the center is dated 9/10/21. The center was due for an updated health inspection in September 2022.
2. The center director confirmed that the health inspection for the center is overdue.

Plan of Correction: The center responded with the following: The center director contacted the health department on 2/13/23 to schedule the annual inspection and was informed that they are backed up due to staffing issues and it will be at least 2-3 weeks before they are able to respond to the center to conduct an inspection.
