1. Reviewed out-of-state background check requirements. Inspector will send link to conduct national sex offender searches.

Conducted an unannounced monitoring inspection at 10:10am. Observed 28 children + 6 direct-care staff + 1 program director. Ratios and supervision were in compliance. Children were engaged in a variety of activities to include: free-play with blocks + toys, art, snack, and outdoor active play. Staff were actively engaged with the children and activities. Classrooms were found to be clean and well supplied with toys and equipment for the children. Artwork made by the children was displayed throughout the facility. The outdoor playground was found to be well maintained. Areas of non-compliance with standards and laws reviewed were found. Standard #: 22.1-289.035-B-4 Description: Based on records reviewed, it was determined that 1 of 4 staff records did not include documentation of a required out-of-state criminal record check completed prior to the 1st day of employment. Staff #5 ? Date of employment was 8/15/22. There was no documentation of the completed background check on-file. Plan of Correction: Teacher no longer on staff. Will note for next hire.
Standard #: 8VAC20-780-240-A Description: Based on records reviewed, it was determined that 1 of 4 staff records did not include documentation of the VDOE orientation course within the first 90-days of employment. Evidence: Staff #5 ? Date of employment was 8/15/22. There was no documentation of the completed training course on-file. Plan of Correction: Teacher no longer on staff.
Standard #: 8VAC20-780-280-B Description: Based on observations made, it was determined that a potentially hazardous substance was not kept in a locked place and was accessible to children. Evidence: A spray cleaning product was found to be sitting on a countertop in the 3 year-old classroom and was within reach of the children present. Plan of Correction: The cleaning product was moved to a locked cabinet during the inspection.
