Technical Assistance:
1. Twenty-four hours of lead teacher training must be documented in a way that clear shows the number of hours the staff received in the various training areas and document the dates the trainings took place. Twenty four hours of training cannot be provided in one day.
2.Makes sure the teachers are not moving the furniture around in the various rooms whereby they are creating blind spots or hiding places for children.
3. Please consistently use the same form to document injuries/accidents. Make sure there is a place to document time the parents were told of the injury.
4. Consultaiton provided on the requirements regarding children with allergies, dietary restrictions and sensitivities.

Comments:
An unannounced monitoring inspection was conducted with the director today from 1:30 pm to 3:45 pm. The director was present with 4 additional staff and 27 children, ages ranging from 11 months to 5 years. The preschoolers were napping during the walkthrough. One child had medication on site. Five staff files and six children's files were reviewed today. Within 10 calendar days please provide documentation that you have sent off for staff C's central registry background check and provide a copy of said background check once it is complete.

Description: Based on a review of five staff files and interview with the director, it was determined that there was no fingerprint background check on file for one staff prior to employment.
Evidence:
Staff D was hired on 8/16/22 and the fingerprint background check was dated 8/17/22.

Description: Based on a review of staff files and an interview with the director, it was determined that there was no central registry finding on file within 30 days of employment.
Evidence:

Description: Based on a review of staff files and interview with the Administrative staff, it was determined that staff were not given orientation training in all of the topic areas before working alone with children and no later than seven days of the date of assuming their job responsibilities.

Staff A, B, C, D and E were missing orientation training in the areas of sudden infant death syndrome , safet sleep practices, shaken baby syndrome, abusive head trauma, coping with distraught children. prevention and response to emergencies due to food and other allergies, and prevention and control of disease.

Description: Based on observation, it was determined that there was not at least six inches of resilient surfacing in the fall zones of the playground equipment.
Evidence:
The Licensing Inspector observed mulch that had degraded to dirt underneath and surrounding the merry -go-round and the 5 seat trike set on the playground. Dirt is not an acceptable resilient surface.
