Technical Assistance:
1. Guidance provided to the vendor regarding her intended move to Prince William County.
2. Forms provided: daily attendance, and transportation permission.

Comments:
3:25-5:20pm Annual unannounced subsidy inspection conducted. Observed 3 children = 2 points with the vendor. Ratios and supervision were in compliance. The children were observed arriving from school, washing hands, eating snack, and reading books. The home was found to be sufficiently supplied with games, drawing supplies, and books for the children. The vendor is currently certified in First Aid/CPR.

Violations:
Standard #: 22VAC40-665-160-B
Description: Based on interview conducted, it was determined that a written record of daily attendance with arrival + departure times of each child as it occurs has not been maintained. Evidence: The vendor said she has not been writing daily attendance, but using the swipe machine receipts instead.

Plan of Correction: The vendor will write attendance on the form everyday.

Standard #: 22VAC40-665-160-C
Description: Based on records reviewed, it was determined that 3 of 3 children's records did not include required information.

Evidence:

1. Child #1 - Parent name(s), home/work addresses and phone numbers; name, address and phone number of emergency contact person; names of persons authorized to pick up the children; written permission for emergency medical care; sick child and medicine policies; and written permission to transport the children in the car.

2. Child #2 - Parent name(s), home/work addresses and phone numbers; name, address and phone number of emergency contact person; names of persons authorized to pick up the children; written permission for emergency medical care; sick child and medicine policies; and written permission to transport the children in the car.

3. Child #3 - Parent name(s), home/work addresses and phone numbers; name, address and phone number of emergency contact person; names of persons authorized to pick up the children; written permission for emergency medical care; sick child and medicine policies; and written permission to transport the children in the car.

Plan of Correction: The vendor said she will have the parent complete the required information and forms.

Standard #: 22VAC40-665-240-C-2
Description: Based on observations made, it was determined that 2 electrical outlets in the upstairs hallway did not have safety covers.

Plan of Correction: The vendor said she will cover the outlets.

Standard #: 63.2-1721.1-B-1
Description: Based on interview conducted and observations made, it was determined that an adult living in the home does not have a completed sworn disclosure statement. Evidence: The vendor said Adult #2, who was sleeping in the home at the time of inspection today, moved into the home on 1/1/20.

Plan of Correction: Adult #1 will complete a sworn disclosure statement in the next 14 days, as long as childcare continues in the home.

Description: Based on interview conducted and observations made, it was determined that an adult living in the home does not have a completed criminal record check. Evidence: The vendor said Adult #2, who was sleeping in the home at the time of inspection today, moved into the home on 1/1/20

Plan of Correction: Adult #2 will be fingerprinted in the next 14 days, as long as childcare remains in the home.

Standard #: 63.2-1721.1-B-3
Description: Based on interview conducted and observations made, it was determined that an adult living in the home does not have a completed central registry check. Evidence: The vendor said Adult #2, who was sleeping in the home at the time of inspection today, moved into the home on 1/1/20

Plan of Correction: Adult #2 will submit a central registry check in the next 14 days, as long as childcare remains in the home.
