Technical Assistance:
1. Reviewed proof of identity form completion requirements.
2. Discussed making sure outlets are covered; hazardous items removed before children arrive each afternoon; and bed linens used to cover the existing linens when children in care sleep there.
3. Advised vendor to keep all records, including attendance, for 5 years after the child leaves care.
4. Information given to vendor about 3 hour health and safety training.

Comments:
2:10-3:25pm Annual unannounced subsidy inspection conducted. There were no children present however, 4 children are enrolled. Currently care is provided in the evening, and it was discussed how the vendor may be able to have a day-shift, as well. The vendor completed the required hours of child development training, and is currently certified in First Aid/CPR. Areas of non-compliance found.

Standard #: 22VAC40-665-210-B
Description: Based on records reviewed, it was determined that 2 of 4 children's records did not include documentation of immunizations every 6 months until the age of 2 years old.

Evidence:

1. Child #3 - Immunizations were dated September 2014, when the child was 21 months old.

2. Child #4 - Immunizations were dated October 2017, when the child was 15 months old.

Plan of Correction: The vendor said she asked for immunizations records last year and did not get them. She will ask again.

Standard #: 22VAC40-665-410-B
Description: Based on interview conducted, it was determined that a record of practice emergency drills has not been kept. Evidence: The vendor said drills have not been written for 2020.

Plan of Correction: Drills will be practiced and written down every month.
