8VAC20-780-560G All food from home, including water bottles if filled at home, will need to be labeled with name and date.

Comments:
An unannounced monitoring inspection was conducted on 02/21/2023 from 1:00pm to 2:50pm. There were 39 children ages 3 years old to age 5 years old supervised by 4 staff. The physical plant, outdoor playground area, programming, attendance, menus, 3 staff records, 4 children?s records, 3 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed in nap time. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-80-A
Description: Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.

Evidence: During inspection in the Busy Bees classroom, there were 15 children present, but the written record reflected there were 0 children present.

Plan of Correction: One class did not have attendance taken at the time of the visit. The other classes did have their attendance sheets completed. Steps have been taken to ensure attendance is taken as soon as the children arrive for preschool.

Standard #: 8VAC20-780-330-B
Description: Based on observation and inspection of the playground, the Center did not to ensure that the required amount of resilient surfacing was under playground equipment with moving parts or climbing apparatus to create a sufficient fall zone.

Evidence:
1)The Preschool playground had 2? of pea gravel in front of the climbing apparatus that was 38? from the ground. The required depth of pea gravel is 6 inches.
2)The Preschool playground had a merry go round that was 26? from the fence on one side and 35? from the fence on another side. The required distance from an obstruction for a fall zone should extend to 6 feet

Plan of Correction: Our playground does have the required amount of resilient material, however, it did need to be redistributed to ensure the depth was proper in all areas.

Our playground tike cycle has a very low fall zone and I was not aware this would need to be 6-feet from the fence. It will be moved to meet this requirement.
