A monitoring inspection was conducted on 03/01/2023A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
1. There was peeling paint on the wall throughout the building.
2. The wooden trim around the hallways and classrooms were chipped and peeling.
3. The floor trim in the school-age room was separated from the wall in some areas.Plan of Correction: Moving forward safety captain and staff will inspect our physical environment meets safety standards daily. Any deficiencies will be reported to management within 24 hours. Management will also inspect through walk throughs to ensure compliance.
Standard #: 8VAC20-780-440-H Description: Based on observation the center did not ensure all requirements were met for the cribs.
Evidence:
In the infant room there were 3 cribs that had more than 1 inch of space between the mattress and the crib.Plan of Correction: Moving forward proper fitting sheets will be put on the mattress to ensure that mattresses are flat within the crib.
