Comments:

A monitoring inspection was completed on March 6, 2023. There were 13 children present with 4 staff supervising and including administrative staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description:	Based on review of staff records and interview, the center did not ensure to obtain a copy of the results of a search of an out of state child abuse and neglect registry. Evidence: In Staff#1's record (date of hire: 09/13/22), there was no results of an out of state central registry found. Interview with staff confirmed the search results not being available for review during the inspection.

Description:	Based on observation, the center did not ensure children's hands shall be washed after toileting and any contact with blood, feces, or urine. Evidence: During the inspection on March 3, 2023, there was observation of Staff#1 diapering a child in the restroom on the diapering area and Child#1's hands were not washed after diapering.
