Comments:
An unannounced on-site renewal inspection was conducted on 03/06/2023. There were 32 children, ages 10 months-6 years, and 5 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, lunch and nap. A total of 5 children?s records and 4 staff records, were reviewed. The inspector discussed the following with staff: balanced mixed age grouping, obtaining an updated declaration of insurance, labeling the crib with child's name and first aid kit. The inspectors arrived at 11:02 AM and departed at 1:45 PM.

Description: Based on review of five children?s records, the center failed to that immunizations for children 2 and under were updated every 6 months.

Evidence:
Child # 5 (DOB 5/22/2020) and documented enrollment date of 10/26/2020 did not have updated immunizations ever six months. The most recent immunization record is 8/25/2020.
