A complaint was received by telephone on February 27, 2023. An unannounced complaint inspection was conducted on February 27, 2023.
There were 51 children in care during the inspection. There were 5 staff present upon arriving to the center. Inspector arrived during naptime so ratios were doubled.
The information gathered during the inspection did support the complaint and therefore the complaint was determined to be valid. Information gathered during the course of the investigation determined non-compliance with applicable standards or law. Violations related and not related to the complaint were documented on the violation notice issued to the provider.
The inspector arrived at 1:40PM and departed at 3:00PM.

Standard #: 8VAC20-780-270-A Complaint related: Yes Description: Based on observation, the center failed to ensure all areas and equipment of the center, shall be maintained in a safe, clean, and operable condition.
Evidence:
Classroom #1 had food crumbs on multiple tables. There were food and drink stains visible on the tiled floor and numerous sticky spots on the floor. Staff #1 stated the room was in that condition when they arrived that morning. Staff #1 stated none of the food or drink stains on the floor or food crumbs came from the children in care that morning.Plan of Correction: Classrooms will be cleaned daily to ensure that they are ready to receive children. Checklist with sign-off will be provided to make sure all tasks are completed with accountability. Staff will also be reminded to provide additional cleaning after meals are served in the classroom and as needed throughout the day in order to provide a clean environment for the children in care.
Standard #: 8VAC20-780-500-B Complaint related: Yes Description: Based on interview and documentation, the center failed to ensure that when a child?s diaper becomes wet or soiled, the child shall be cleaned and changed immediately.
Evidence:
Staff #1 stated that eighteen children in diapers were in care before 10:45 AM on the day of the inspection. Children in diapers began arriving to the center at approximately 6:30 AM. Staff #1 stated they were aware that multiple children had bowel movements and sagging wet diapers but could not change the children because they were out of diapers and wipes. Staff began changing children at approximately 10:45 am. On the Tadpole App, diaper changing was logged at 10:45 AM for 18 children. Staff #1 stated some of the children had bowel movement stuck to their bottoms which had to be scrubbed off.Plan of Correction: During this time, this location was being utilized to care for the children from another site due to recent water damage/ remodel. In the future, should children in diapers be on-site, staff will be required to inventory required items needed prior to the next day of operation to ensure no delays in diapering or any other aspect of the day.
Standard #: 8VAC20-780-560-G Complaint related: No Description: Based on observation, the center failed to ensure that when food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.
Evidence:
In Classroom #1, there was a bag of food in a cubby. The bag of food was unlabeled and had no date.Plan of Correction: Staff will be reminded to properly label all food that is bought from home. It will be marked with the child's name and date to ensure that it is identifiable.
