Comments:
An announced initial inspection was conducted on March 13, 2023. There were no children present. The physical plant, staff records, emergency supplies, toys, games and age appropriate equipment were all reviewed. The playground was also inspected. Cribs are all new and meet current requirements. Violations:
Standard #: 8VAC20-780-40-H
Description: Based on a review of documents, the center failed to maintain public liability insurance for bodily injury for each center site with a minimum limit of at least $500,000 each occurrence and with a minimum limit of $500,000.

Evidence:
The center records did not contain documentation of current insurance coverage

Plan of Correction: I will get the new insurance verification and will email it to the inspector.

Standard #: 8VAC20-780-240-C
Description: Based on a review of staff records, the center failed to ensure that staff complete Recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by the Code of Virginia.

Evidence:
Staff Member #1, #2, #3, #4 and #5's records do not have documentation of the completion of Recognizing Child Abuse and Neglect.

Plan of Correction: I will ensure that all staff have completed the Recognizing Child Abuse and Neglect training and place documentation in staff records.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center failed to ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:
In the Toddler C classroom, the rubber baseboard is not completely attached to the wall.

Plan of Correction: Baseboard was repaired during the time the inspector was on site.

Toilet was repaired during the time the inspector was on site.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center failed to ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In Toddler B,. Toddler C, Classroom 2A, 2B and 2C, cleaning products were not in a locked place using a locking method that prevents access by children.

Plan of Correction: Cleaning supplies were moved to locked cabinet during the time the inspector was on site.
