Comments:

The Licensing Inspector initiated a monitoring inspection on 02/16/2023 and completed the inspection on 3/10/2023. The inspector was on site from 3:10pm until 4:10pmon 2/16/2023. There were thirty children under the care of three staff. The ages of the children ranged from five to eleven years old. The children were observed during indoor free play, and snack time. Three children?s record and two staff records were reviewed. Description:	Based on record review and interview, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the competition of a fingerprint background check as required by the Code of Virginia. Evidence: Staff #1, hire date of 8/1/2017 had no fingerprint eligibility letter on record during the inspection on 2/16/2023.

Description:	Based on record review, the center failed to obtain a completed Sworn Statement prior to first day of employment for staff member.
