Technical Assistance:
Discussed director qualification documentation and bathroom partitions rusting.

Comments:
A monitoring inspection was initiated on March 3, 2023 and concluded on March 15, 2023. There were fifty-five chilen present with 12 staff superviing. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and six staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on staff interview and record review, the licensee did not ensure there was a out state child abuse and negelct for staff who lived outside Virginia in the past 5 years.

Evidence:
Staff #1 hired in 08/2022. The center was unable to provide documentation of Staff #1 Out of State Child abuse and neglect registry for New York.

Plan of Correction: NYS background checks will be completed and submitted on or before 3/21/23. Moving forward all employee will have out of state background checks completed before hiring. DM will review for compliance during all school visits.

Standard #: 8VAC20-780-40-M
Description: Based on observation and staff interview, the licensee did not ensure the center maintained a allergy list accessible to all staff who work with children . The list should be dated and kept confidential in each room or area where children are present.

Evidence:
Staff in the 2 year old classroom was unable to locate the allergy list.

Plan of Correction: Update allergy postings were posted the day of the visit. Moving forward, the school safety captain and center management will both check monthly for compliance.

Standard #: 8VAC20-780-260-A
Description: Based on record review and staff interview, the licensee did not ensure there was annual fire inspection completed yearly.

Evidence:
The center did not provide documentation of the date of most recent fire inspection. Center did provide a schedule date of fire inspeciton for 03/09/2023.

Plan of Correction: Fire inspection has been requested. Center Director will calendar to ensure compliance, annually.

Standard #: 8VAC20-780-260-B
Description: Based on documentaiton and staff interview, the licensee did not ensure a annual health inspection is conducted annually.

Evidence: The center did not provide documentation of the most recent Health inspection. Center stated a Health inspection is schedule to be conducted March 30, 2023.

Plan of Correction: Health inspection has been requested. Center Director will calendar this to ensure compliance, annually.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the licensee did not ensure the diapering surface is used only for diapering.

Evidence:
In the Toddler classroom and 2 year old classroom , there were tablets sitting on the diapering surface.

In the 2 year old classroom , there was also a clipboard on the diapering surface.

Plan of Correction: All staff in diapering rooms will review safety protocols for changing tables and diapering on or before 3/22/23. Management and the safety captain will observe daily for compliance.

Standard #: 8VAC20-820-120-E-1
Description: Based on observation and staff interview, the licensee did not ensure the most recent license was posted at each public entrance of the facility.

Evidence: Staff confirmed they have did not have it posted at public entrance. The center had the building painted and did not posted back up and had it in a box.

Plan of Correction: The license was posted the day of the visit. Due to the school being painted, it had been taken down. Center Director will ensure that it is always posted on a shelf display, if unable to hang.

Standard #: 8VAC20-820-120-E-2
Description: Based on observation and staff interview, the licensee did not enusure the most recent inspection was posted .

Evidence: The center had the facility painted and did not post the most recent inspection. Confirmed by the staff.

Plan of Correction: The license was posted the day of the visit. Due to the school being painted, it had been taken down. Center Director will ensure that it is always posted on a shelf display, if unable to hang.
