Comments:
An unannounced monitoring inspection was initiated and concluded on 3/29/2023. The inspector was on site from approximately 8:50 am-10:32 am. There were 13 children present, ranging in ages from 3 to 4 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition and background checks. A total of 5 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-540-D
Description: Based on observation and interview, the center did not ensure that the required emergency supplies were available on field trips.

Evidence:
1. The center's bus did not have an ice pack or cooling agent.
2. Administration acknowledged that the ice pack or cooling agent was not on the bus and the bus had been in use without having it.

Plan of Correction: A box of ice packs has been included with the first aid kit on the center's field trip bus.

Standard #: 8VAC20-780-550-K
Description: Based on observation and interview the center did not ensure to prepare a document containing local emergency contact information, potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business such as field trips or pick up or drop off of children to or from schools etc. This document must be kept in vehicles that centers use to transport children to and from the center.

Evidence:
1. The center did not have a document with the required shelter, hospital and route information on the center's bus as required.
2. Administration acknowledged that the documents had not been created and kept on the vehicle.

Plan of Correction: The information will be prepared, printed and placed in the field trip binder by 4/7/23.
