Comments:
An unannounced monitoring inspection was conducted with the director on March 27, 2023 beginning at 5:20PM and ending at 5:58PM. There were 7 school aged children in the care of 4 direct care staff. The children were observed doing a video project and interacting with staff. Five children?s records were reviewed and in compliance. Five staff records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. There were no medications on site to be reviewed. The menu, daily attendance, and emergency drill log were reviewed. The most recent evacuation drill was documented as 3/27/23. The most recent shelter-in-place drill was documented as 11/17/22. The most recent health inspection was dated September 9, 2022. The most recent fire inspection was dated July 21, 2021.

Standard #: 8VAC20-770-60-C-2
Description: Based on a review of 3 staff records, the facility failed to obtain documentation of a central
registry finding within 30 days of employment for all staff. Evidence: Staff A (start date
1/17/23) and staff B (start date 1/25/23) were missing documentation of a central registry finding in their records.

Plan of Correction: The central registry background check was completed for both staff members
but documentation was not present during the inspection. Documentation of one staff's central registry was provided at the conclusion of the inspection. Documentation of the central registry findings will be printed and placed in the staff's records.
