Technical Assistance:
Discussed written permission and requirement to complete the annual Health and Safety Update before March, 2020
Reminder to Keep Current Vaccines on hand for pet.

Comments:
An unannounced inspection was conducted from 2:40-3:50. The Vendor was present with 1 child for a total of 2 points.The Vendor is current in First Aid/CPR and has documentation of attending trainings. The back room was observed and consultation was provided on storage of toys and materials. The Vendor stated the back yard is not used and they walk to the playground. Description: Based on record review and observation, it was determined the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. Child present today and there is no attendance recorded for today. The last few days are also not filled in for the other two children but vendor stated they have been coming after school.

Plan of Correction: I will write down.

Standard #: 22VAC40-665-170-B
Description: Based on review of children's information, it was determined that all required information were not maintained.
Evidence:
1. All 3 children's information did not include written permission for field trips and the Vendor stated she takes them to the park .
2. Child 1 was missing name, address, and phone number of parent's employment.
3. Child 2 and 3 were missing written authorization to obtain emergency medical care.
4. Child 3 is a sibling and was missing if the child had any allergies.

Plan of Correction: I will have families sign the field trip permission or have them write and sign that they give me permission. I will have parents update information.

Standard #: 22VAC40-665-410-A-3
Description: Based on interview and record review it was determined the vendor did not practice an lockdown procedure in the last year.
Evidence:
1. The Vendor had an Emergency log and the lockdown portion was blank. The Vendor said she didn't have one.

Plan of Correction: I will do one.
