Comments:
An unannounced monitoring inspection was conducted on 4/11/2023 from 11:00am to 12:45pm with the Director. There were 49 children in care, ranging in age from 14-months to five-years-old, supervised by 9 staff. The children were observed eating lunch, having diapers changed, and getting ready for nap. Five child records and three staff records were reviewed. The Center has 15 staff with current certification in CPR and First Aid as well as trained in Daily Health Observation. Seven medications and authorization forms were reviewed and the center has five staff current in Medication Administration Training (MAT). The attendance, menu, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 8/2/22 and the most recent Health Inspection on file was dated 12/7/22. Lunch served was grilled cheese sandwiches, corn, applesauce, and milk. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 4/18/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-500-B
Description: Based on observation, the center did not ensure that diapering areas had a nonabsorbent surface for diapering or changing. Evidence: 1. In the ?Teddy Bears? class for children 16-months to two-years-old, the changing pad had a large hole in the top corner, making the surface absorbent. 2. In the ?Frogs? class for children two-years-old, the changing pad had approximately six small tears across the surface, making it absorbent.

Plan of Correction: Both changing pads from Teddy Bears and Frogs were removed. New changing pads are placed.

Standard #: 8VAC20-780-510-I
Description: Based on review of medication, the center did not ensure that the requirements were met to be able to administer prescription medication to a child. Evidence: Two emergency medications for Child A were observed on-site on 4/11/23 that were not maintained in their original, labeled container.

Plan of Correction: Original labeled container is requested from parent and the medicine is secured in labelled box.

Standard #: 8VAC20-780-570-B
Description: Based on observation, the center did not ensure that bottles were not used while children were in their designated sleeping location. Evidence: On 4/11/23 in the ?Teddy Bears? classroom for children 16-months to two-years-old, a child was observed on their cot with a bottle. A staff member then placed another child on their cot and handed the child a bottle.

Plan of Correction: Staff members are redirected and are instructed to do the Safe Sleep training.
