An annual SHSI inspection was conducted today, the vendor is present with one child in care totaling 3 points. The child was observed playing and eating lunch during the inspection. Lots of interaction between the vendor and the children. The child and vendor was observed in the living room upstairs, the vendor also has a large playroom in the basement. The vendor provides food and takes the children outside to play. Three children attend after school.This inspection was started at 11:00 and ended at 12:30. Standard #: 22VAC40-665-160-B Description: Based on the review of the the vendors attendance record, the vendor failed to keep an up to date record of the arrival and departure of the children in care.
Evidence: The vendor does not have up to date attendance record , the last time she took attendance was February 26, 2020.Plan of Correction: I will make sure I take attendance every day.
Standard #: 22VAC40-665-170-B Description: Based on the review of 4 children's records , it was determined that the vendor failed to get some of the required information to make the children's records complete.
Evidence:
Child 2,3 and 4 are missing the emergency contact address and the employment information for the parents.Plan of Correction: I will ask the parents for the information.
Standard #: 22VAC40-665-240-C-2 Description: Based on the review of the physical plant the vendor failed to cover electrical outlets with protective covers.
Evidence: 4 electrical outlets are missing covers . three are located in the living room one in the basement area used for care.Plan of Correction: I will make sure I put the protective covers on.
Standard #: 22VAC40-665-410-B Description: Based on the interview with the vendor, the vendor was not able to locate the drills recorded for the last 12 months.
Evidence: The vendor found several other records from previous years but could not locate the drill from the last year.Plan of Correction: I will find the records and send them to you
