A monitoring inspection was conducted on 05/02/2023. There were a total of 53 children with 19 staff members present. Children were observed during lunch time, engaged in group activities and handwashing. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 7 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-780-160-A Description: Based on a review of staff records, the center did not ensure each staff member submitted documentation of a negative tuberculosis screening prior to coming into contact with children.
Evidence:
Staff #2 (date of hire 5/1/2023) did not have a TB test on file.
Staff #6 (date of hire 12/20/2022) had a TB test dated 12/30/2022.
Staff #7 (date of hire 11/7/2022) had a TB test dated 11/9/2022.Plan of Correction: Candidates will have a pre-employment meeting before their start date to collect TB test results and coordinate the hire date after receiving negative test results.
Standard #: 8VAC20-780-280-B Description: Based on observation, the center did not ensure hazardous substances such as cleaning materials be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
In room #208 2 bottles of bleach water and 1 bottle of handwashing detergent were in an upper unlocked cabinet above the children's sink.Plan of Correction: Will ensure all cabinets containing hazardous substances are locked.
