Comments:
The vendor is present with 1 child during this annual health and safety inspection. The inspection is unannounced. The vendor has a total of 4 points today. The child is playing and reading with the vendor during the inspection. The vendor and child are interacting during the inspection. The space used for care has lots of supplies . children bring food from home, the vendor does transport the children. This inspection started at 11:20 and ended at 1:15. The vendor's husband was present during the inspection and discussed the child care in the home. Please call me at 703-203-8659 if you have any questions regarding this inspection.

Standard #: 22VAC40-665-170-B
Description: Based on the review of the vendors records it was determined that the vendor failed to have the permission to transport children in the child's file.
Evidence: When the inspector arrived at the home the vendor was returning from a trip to CVS, the vendor does not have permission for transportation for the child in care, child A.

Plan of Correction: I will ask the parent to fill out the paper you have given me today.

Standard #: 22VAC40-665-240-A
Description: Based on the review of the physical plant the vendor failed to keep the home in a clean condition.
Evidence: The vendor had a water leak and has holes in the ceiling in the kitchen and bathroom that are used for care and in the basement . According to interview with the vendor the leak happened two weeks ago. The fence in the backyard that is between the vendors house and the town house next door has fallen down exposing broken glass in the neighbors yard.

Plan of Correction: I will ask my uncle to fix the drywall and i will discuss the fence with my neighbor. I do not use the backyard for play we play at the playground next door

Standard #: 22VAC40-665-390-A
Description: Based on the review of the vendors emergency supplies it was determined that the vendor is missing one of the items needed to make the emergency supplies complete.
Evidence: The vendor is missing a digital Thermometer

Plan of Correction: I will buy one at the store.
