Comments:
A monitoring inspection was conducted on May 9, 2023 from approximately 9:30 AM to 3:05 PM. There were 156 children present in the preschool, ranging in ages from two years to five years, with 34 staff supervising. The center director was available throughout the inspection to assist and answer any questions.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, transportation and field trips, special care and emergencies and nutrition.

During the inspection, the children were observed having outdoor play time, preparing for and having lunch. One classroom was observed preparing to the leave the center for a field trip. Staff were observed having positive interactions with the children. All classrooms and bathrooms used by the children were inspected. The required postings were reviewed and found to be in compliance. The center is equipped with toys and supplies and items were available to the children.

A total of 10 child records and 10 staff records were reviewed. Medication is administered, and medication, authorization forms and allergy plans were reviewed. The annual staff training hours and annual fire and health inspections were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable.

Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on staff record review, the center did not ensure 3 out of 5 staff completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:

Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.

Evidence:
1. In classroom #3, there was rust on the bottom of the wall directly behind the toilet used by children.
2. In classroom #4 there was peeling paint on the bottom of the wall directly behind the toilet used by children.
3. In classroom #6 there was peeling paint on the wall directly underneath the hand soap dispenser used by children.
