Comments:
The licensing inspector conducted an unannounced monitoring inspection on 05/17/2023 in response to a self report made to the licensing office regarding allegations in the area of Programs. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.011-F
Description: Based on observation the center did not have the results of the last inspection posted in a conspicuous place on the licensed premises.

Evidence:
The center did not have the results of the last inspection posted anywhere in the building.

Plan of Correction: Plan of Correction: Was corrected on 06/01/2023. Moving forward Director will
ensure most current violation is posted after it is sent by licensing. DM and
operational support staff will audit to make sure current violation is posted when
visiting the building.

Standard #: 8VAC20-780-40-E
Description: Based on interviews and a record review the center did not ensure the center's activities services, and facilities were maintained in compliance with these standards and the center's own policies and procedures that are required by these standards.

Evidence:
The center's Playground Policy states under Stage 5: Supervision, " Staff must anticipate children's movements and ensure visual supervision of all children." Staff #1 did not adhere to this policy when she took a group of school aged children to the playground on 5/10/23, when child #1 sustained an injury to his face by rocks thrown by child #2.

a. During the interview Staff #1, who was responsible for supervising the group of school-age children on the playground, stated she was aware child #1 and child #2 do not get along. Staff #1 further stated, at the time of the incident she did not see the 2 children engaging in rock throwing and did not see how the injury occurred to child #1's face.

Plan of Correction: Plan of Correction: Will be corrected by 06/09/2023. Staff # 1 was retrained on child
supervision and playground policy to ensure visual supervision of all children. Pea
gravel on playground was replaced with mulch. All staff are trained during
orientation on proper supervision of children prior to being in ratio. Management
will do periodic inspections and ensure a manager is available throughout the day
if staff need assistance.

Standard #: 8VAC20-780-340-A
Description: Based on interviews and record review the center did not ensure the care, protection and guidance of children when staff are supervising them.

Evidence:
1. On 05/10/2023 at approximately 5:10pm a 6 year-old child #1 sustained welting and scratches on the face after he was hit in the face with rocks thrown by another child. During outdoor play, Staff #1, who was responsible for supervising the group of school age children which included child#1, stated she did not see the incident occur. Staff #1 further stated she only heard child #1 crying after he was hit in the face.

a. Staff #2, who was supervising a separate group of children in another area of the playground said she saw child#1 and child#2 throwing rocks at each other, however did not intervene. It was not until after child #1 came to her crying after getting hit in the face, did Staff #2 bring the situation to Staff #1's attention.

2. A staff member in the infant room was observed lifting infants up by the upper arm without supporting the head when initially lifted.

Plan of Correction: Plan of Correction: Will be corrected by 06/09/2023. Staff # 1 & # 2 was retrained
on child supervision and playground policy to ensure visual supervision of all
children. Pea gravel on playground was replaced with mulch. Infant room staff
were retrained on proper head support for infants. All staff are trained during
orientation on proper supervision of children prior to being in ratio. All infant staff will be trained on proper head support before being placed in ratio. Management will do periodic inspections and ensure a manager is available throughout the day if staff need assistance.

Standard #: 8VAC20-780-500-A
Description: Based on observation the center did not ensure staff washed their hands when required.
Evidence:

1. A staff member in the infant room did not wash her hands after cleaning-up a child's spit-up on the floor or when she cleaned a child's nose that had green fluid from her nose.

2. A staff member did not wash her or the infant's hands before feeding the child a bottle.

Plan of Correction: Plan of Correction: Will be corrected by 06/09/2023. Infant staff was retrained on
hand washing policy. All infant staff will be trained on handwashing policy before
being placed in ratio. Management will do periodic inspections and ensure a
manager is available throughout the day if staff need assistance

Standard #: 8VAC20-780-500-B
Description: Based on observation the center did not ensure when a child's clothing or diaper becomes wet or soiled, the child shall be cleaned and changed immediately.

Evidence:
1. The board in the infant room listed child #1 as having her diaper changed at 11:03am and due for the next change at 1:03pm. As approximately 11:43am, child #1 was observed with a saggy diaper. Staff placed child#1 on the changing table at approximately 11:50am to wipe her nose, however did not check or change her diaper.

2. The board in the infant room listed child #2 as having a diaper change at 9:53am and due by 11:53am. Child #2 was observed with a saggy diaper and the blue stripe on the diaper indicating it is wet was visible through the white onesie the child was wearing. A staff member put a sleep sack on child #2 at approximately 11:47am and placed her in the crib without checking or changing the child's wet diaper.

Plan of Correction: Plan of Correction: Will be corrected by 06/09/2023. Infant staff was retrained on
diaper changing policy. All infant staff will be trained on diaper changing policy
before being placed in ratio. Management will do periodic inspections and ensure
a manager is available throughout the day if staff need assistance.
