Comments:
An unannounced monitoring inspection was conducted on June 5, 2023 with center director. There were 59 children present, ranging in ages from 2 months to 6 years, with 11 staff supervising. The inspector observed children participating in circle time and playing with various age-appropriate materials. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-780-60-A
Description: Based on the review of five (5) children?s records, the center did not ensure that all children?s records were complete.
Evidence: Child #5?s (DOE 6/1/2021) record, did not contain documentation of child updates and confirmation of up-to-date information.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence: Chipped and peeling paint was observed around the wooden window frames in 3 out of 7 classrooms (classroom #2, classroom #4 and classroom #7).

Description: Based on interview and record review, the center did not implement a monthly evacuation drill.
Evidence:
1. Documentation for evacuation drills indicated that an evacuation drill for the month of May was not condcuted.
2. Staff #5 confirmed that a monthly evacuation drill was conducted for the month of May but had not been documented.
