Standard #: 22.1-289.035-B-4 Description: Based on staff record review and interview, the center failed to obtain an Out of State child abuse and neglect search request for a new staff member.
Evidence:
Staff #4 began employment on 1/7/2023. Staff #4 has lived in another state within the past five years. The record for Staff #4 does not have a child abuse and neglect search request for the state of prior residence. Staff #7 stated it has been sent but not received back or checked on with the last 30 days.Plan of Correction: HR will be send out an email to check on missing background check.
Standard #: 8VAC20-780-70 Description: Based on staff record review, the center failed to ensure that each staff member had two or more references checked before employment or volunteering.
Evidence: Staff #1, who was hired on 08/06/2021 had reference checks done on 8/19/2021.
Staff #4, who was hired on 01/07/2023 had reference checks done on 1/09/2023.Plan of Correction: All references will be checked prior to first day moving forward.
