Standard #: 8VAC20-770-60-C-2 Description: Based on review of three staff records, the facility failed to have documentation of a completed search of the central registry (CPS check) on file for each staff within the first 30 days of employment. Evidence: Staff C (date of employment 11/14/2022) did not have a completed search of the central registry on file. There was no documentation of having submitted the search of the central registry within 7 days of employment as required. Plan of Correction: The director will follow up with Central Registry to get a copy of the CPS check.
Standard #: 8VAC20-780-160-C Description: Based on a review of three staff records, the facility failed to have a repeat tuberculosis (TB) test or screening at least every two years from the date of the initial screening. Evidence: Staff B had a tuberculosis test that expired 3/16/2023. There was no documentation of a current TB test or screening. Plan of Correction: Staff B will provide documentation of a current TB test or screening.
